This Privacy Policy applies to the website (“Website”) for the King County Mass Vaccination Site– Redmond in connection with the Microsoft CDC COVID 19 Vaccination Programs (the “Programs”). This Policy governs collection, usage, and disclosure of your information via the Website.  

The King County Mass Vaccination Site – Redmond is a collaboration between Microsoft Corporation, Overlake Hospital Medical Center (“Overlake,” “our,” "we,” or "us"), and King County Public Hospital District No. 2 d/b/a EvergreenHealth (“EvergreenHealth,” “our,” "we,” or “us”). Each of these entities provide certain services in support of the Programs.  

Personal Information Collected By the Website:

To schedule a vaccination appointment through the Website, you will need to input certain “Personal Information,” which is information that identifies you. Any information submitted by you through the Website is for the exclusive use of Overlake, EvergreenHealth, and their contractors that are involved in the operation of the Programs, and solely for the purpose of operating the Programs.

Use of Information Collected:

The Personal Information collected will be used by Overlake and EvergreenHealth to improve your experience with the Website, to provide you with a vaccination appointment in connection with the Programs, and to communicate with you about information that you request. Additionally, your Personal Information may be used to:

  • Respond to your questions about the Website.
  • Administer your Website account.
  • Provide information to third parties, including health care providers, as necessary to administer your scheduled vaccination.
  • Reporting to the Washington Department of Health

Sharing of Information with Third Parties:

Overlake and EvergreenHealth will not rent nor sell your Personal Information. Overlake and EvergreenHealth will not otherwise disclose your Personal Information to unrelated third parties without your consent, except as stated below:

  • Overlake and EvergreenHealth may disclose Personal Information to contracted service providers for the purposes of the operations of the Website and Programs, as permitted by applicable law.  
  • To the extent permitted by law, Overlake and EvergreenHealth will disclose Personal Information to government authorities, including the Washington Department of Health.  We will also disclose Personal Information to third parties pursuant to a legal request, subpoena, or other legal process.  

Security of Personal Information:

The Website has commercially reasonable safeguards in place to help protect the Personal Information you provide from loss, misuse, and unauthorized access, disclosure, alteration, and destruction. Although Overlake and EvergreenHealth attempt to protect the Personal Information in our possession, no security system is perfect, and Overlake and EvergreenHealth cannot promise that your Personal Information will remain absolutely secure in all circumstances.

Rights to Personal Information:

You have the right to access and correct certain Personal Information you provide through the Website. For example, your email address is required to use the Website, so you will have the opportunity to update your email account logging into your account and updating your email address. You may also limit the use of your Personal Information by unsubscribing from any emails that are sent to you by following the instructions on the emails. Please note that some information may remain in Overlake’s and EvergreenHealth’s records even after you request deletion of your Personal Information, to the extent permitted by applicable laws.  

Scheduling and administering of the vaccine under the Programs do not create a patient-provider relationship between you and Overlake, Evergreen, or any other licensed health care providers, thus this policy does not provide you with rights related to your protected health information, as defined in the Health Insurance Portability and Accountability Act of 1996, including health information related to your vaccinations.  If scheduled, your vaccination will be provided by a licensed health care provider affiliated with EvergreenHealth, Overlake, or another health care provider entity, and your rights related any health information created in connection with health care services rendered by a provider of one these entities is set forth in the specific entity’s applicable Notice of Privacy Practices. Please note that neither the provider administering your vaccination, nor the entity affiliated with the provider, nor EvergreenHealth, nor Overlake will maintain a medical record of your vaccination. You can, however, obtain a copy of your COVID-19 vaccine record from the Washington State Department of Health at:  https://www.doh.wa.gov/YouandYourFamily/immunization/ChildProfileHealthPromotion/ForParents/AccessyourFamilysImmunizationlnformation.  

Tracking and Collection of Anonymous Information:

We keep track of visits to our site. This application tells us how many visits are made, the date and time of the visit, your computer's Internet Protocol (IP) address, the uniform resource locator (URL) of the website, and operating system you use. This information is solely utilized to evaluate the effectiveness of the Website.

Cookies:

We use "cookies" on this Website. A cookie is a piece of data stored on a visitor's hard drive to help us improve the experience of repeat visitors. Cookies enable us to track visitors to enhance the experience on the Website.

Children:

The Website is not intended for children under 13 years of age and Personal Information is not knowingly collected from children under the age of 13. If we are aware that a child under 13 has submitted Personal Information through the Website without verification of parental consent, we will delete this information.

If we make revisions that change the way we collect, use, or share Personal Information, we will post those changes in this Privacy Policy.
